The London InterCommunity Health Centre provides inclusive and equitable health and social services to those who experience barriers to care and we foster the active participation of individuals and the communities that we serve. We invite applications for the following:

Mental Health Specialist
Permanent full-time or part-time

You will provide care to internally referred clients with a broad range of mental health concerns including serious mental illness, addictions, concurrent disorders and trauma. You will work as a member of an interdisciplinary team and have experience in crisis intervention, clinical assessment, treatment planning and implementation and case co-ordination, all within a recovery framework. You will also participate in program planning, quality improvement and program evaluation.

You have a degree in a relevant health care discipline (e.g. nursing, social work, occupational therapy) and four years’ experience working with individuals with serious mental illness. You have current registration in good standing with an appropriate regulatory college under the Regulated Health Professionals Act; education and experience in group facilitation; strong knowledge of the issues related to the social determinants of health and cultural competency. Experience in a primary health setting and fluency in a second language reflecting the local community is an asset.

 

Case Manager
Permanent part-time

In this new role you will provide care to internally referred clients with a broad range of mental health concerns including serious mental illness, addictions, concurrent disorders and trauma. As a member of an interdisciplinary care team your work will include crisis intervention, assessment and case management – all within a recovery framework.

You have a degree in a relevant health care discipline (e.g. nursing, social work, occupational therapy) and current registration in good standing with the appropriate regulatory college under the Regulated Health Professional Act. Your minimum three years’ of clinical experience working with people with severe mental illness is complimented by formal training and a demonstrated knowledge of concurrent disorders, harm reduction and evidence-based treatment models. You have strong skills in group facilitation and experience developing and delivering presentations and contributing to capacity building initiatives. Certification in Motivational Interviewing is an asset as is a integrated knowledge of mental health and addictions within a rehabilitation framework.
