Full Time 12 Month Contract, with possibility of 12 Month extension

Community Health Centres (CHCs) are non-profit, community-governed organizations that provide primary health care, health promotion and community development services, using interdisciplinary teams of health providers. CHCs are sponsored and managed by incorporated non-profit community boards and funded by the Ministry of Health and Long Term Care. There are currently 75 CHCs in the Province of Ontario

The successful candidate will report to London InterCommunity Health Centre but will be responsible for decision support activities across South Western Ontario (ESC, SW, HNHB and WW LHINs), as well as work with the Performance Management Committee of the Association of Ontario Health Centres.

The Regional Decision Support Specialist (RDSS) will be responsible for:

Facilitating and promoting evidence based planning and decision making in CHCs in the South Western Ontario region;

Becoming the sector subject matter expert (as part of the RDSS team) on particular knowledge areas related to the use of, and data elements in primary care, community based health services, health equity, and others;

Facilitating CHCs in meeting their accountability responsibilities through indicator development, data management, analytics and change facilitation through coaching;

Working with existing data management staff to build capacity for enabling local decision support activities;

Being an integrated member of provincial working groups that advance the evaluation of the CHC Model of Health and Well-being as well as the development and utilization of performance indicators;

Working closely with, and advising the LHINs on performance and data-related issues;

Advising on target setting for MSAA and performance related monitoring;

Working on population-based planning approaches;

Design, participation and providing insight into program evaluation activities;

Participation in virtual and in-person teams environments with a strong focus on collaboration with three (3) other provincial RDSSs and a provincial Research and Evaluation Lead;

Presentation of recommendations to all levels of the organization including Boards of Directors, Executive Directors, Leadership Teams and within clinical and health service teams.

Required Qualifications and Skills

University degree in Health Administration and Planning, Health Informatics, Information Management, or Business;

Masters or PhD in Epidemiology would be considered a strong asset;

Three (3) to five (5) years experience in a health or social services environment, specifically working in planning and/or data management;

Quality Improvement, Lean/Six Sigma knowledge and experience is an asset;

Strength and experience with literature searches for best practices and developing models to describe performance management activities;

A thorough understanding of community-based, multidisciplinary team base approach to health care delivery;

Demonstrated understanding of the role and importance of decision support in an organization;

Exceptional analytical and problem solving skills to facilitate creative solutions;

A demonstrated ability to build trusting relationships and influence others to a course of action;

Strong project management skills;

Excellent verbal and written communication and presentation skills with a strong attention to detail.
