Registered Dietitian, Diabetes Program
Permanent Full-time
You will work as part of a multi-disciplinary team to deliver diabetes programs to clients of the Health Centre as well as to the broader community. You are interested in fostering collaboration and partnerships for the ongoing development, delivery and evaluation of community-based diabetes programming. You are also interested in working with ethno-cultural communities and those living with concurrent chronic disorders. Experience in group facilitation and knowledge of adult education methods are considered assets.
You have an appropriate degree and current registration with the College of Dietitians of Ontario. A minimum two years’ clinical experience in a primary care setting is preferred. Certification in Diabetes Education would be preferred; however, this would also be an excellent opportunity to work toward certification. Experience in a diabetes education program and /or working with ethno-cultural communities is considered an asset.
Primary Health Care Nurse Practitioner, RN(EC)
Permanent, 4 days/week
We are seeking a dynamic Primary Health Care Nurse Practitioner to join a clinical team dedicated to providing health care to individuals who are homeless or at-risk of homelessness. This position offers a unique opportunity to make a difference by facilitating change with people significantly marginalized by poverty, addiction, mental health and housing issues. The Nurse Practitioner works as a member of an interdisciplinary primary care team of physicians and nurses. This team, in turn, is supported by social workers, dietitians, outreach workers, community developers and support staff specializing in the areas of Mental Health Care, Children, Youth & Families, Immigrants & Ethno-cultural Communities, Poverty & Homelessness and Diabetes & Chronic Diseases. The Health Centre operates an anonymous HIV testing clinic on site and has recently developed a community partnership resulting in a new four-person Hepatitis C Care Team.
Qualifications:
- Bachelor of Science in Nursing or Master’s in Nursing
- Primary Health Care Nurse Practitioner Certificate
- Registered Nurse in the Extended Class in good standing with the Ontario College of Nurses
- Understanding of the social determinants of health
- Commitment to the underlying principles of harm reduction
- Experience working with persons living with mental illness and/or addictions
- Demonstrated ability to work effectively in a team environment
- Proficiency in the use of electronic medical records
- Excellent written and oral communication skills
- Membership with the Nurse Practitioners Association of Ontario
The successful candidate has the opportunity to initiate, participate and /or lead programs, presentations, evaluations and other system changes to continue the enhancement of interdisciplinary care for vulnerable persons.
