Systems Navigation provides support for both potential and existing Health Centre clients. Our Navigators respect your privacy and are trained to handle all confidential information carefully.

BECOMING A CLIENT: A Systems Navigator will provide a comprehensive assessment of your strengths and needs across all areas of your life and then assess those needs against the Health Centre’s criteria for accepting new clients. After this process, they will provide you with one or more of the following:

 

 

  • Referrals to appropriate Health Centre programs and services
  • Information on external referrals
  • Help connecting with other health care services
  • Direction to other community resources

EXISTING CLIENTS: One of the most common services is helping clients navigate internal and external services, like choosing the appropriate programs and services, and providing support to those looking for basic needs, including:

  • Social benefits (OW, CPP, ODSP applications)
  • Medication coverage (Trillium)
  • Housing
  • Navigation of various social systems such as
      • Hospitals
      • Schools
      • Interpretation services
      • Learning bonds
      • Immigration

A Systems Navigator can also provide registered clients with a comprehensive assessment of additional needs and offer potential referrals to other, un-accessed Health Centre programs and services. They will connect clients, staff and community residents with information on community resources, and liaison with external community groups.
