Information for Primary Health Care Providers (Trans Health Referral)

Thank you for your interest in referring your patient to the Trans and Non Binary Health Care Program at London InterCommunity Health Centre.

Our Program operates at a Primacy Care level, focused specifically on Gender Affirming Care. Medical assistance through hormonal management and surgical referral options including pre-surgical readiness assessments, and postoperative care are available.   Mental Health (Counselling) supports are available as they relate to gender identity and transition related goals. They may involve exploration of gender identity; coming out to family, employment, and/or society; navigating relationships; issues related to gender dysphoria and coping with minority stress

Our Team consists of MD, Nurse Practitioner, Specialized Registered Nurse and MSW Social worker- all have received extensive training through Rainbow Health Ontario (Sherbourne Health Centre) and the World Professional Association of Transgender Health (WPATH).

We operate at a primary care level using an informed consent model

EXAMPLES OF EXCLUSIONARY FACTORS WHICH ARE BEYOND OUR SCOPE OF CARE

COMORBID MENTAL HEALTH CONDITIONS
      • Psychosis
      • Impaired Executive Function
      • Dissociative Identity Disorder
      • Acute Suicidality
      • Other unmanaged cognitive impairments

We do acknowledge that this cohort of patients are prone to depression and anxiety, and there is a large prevalence neuro divergence such as ADHD or Autism Spectrum Disorder. These are not exclusionary by their existence, but are not aspects of care that can be managed by us concurrently. Adjunct providers should be included in circle of care.

SPECIALTY SERVICES

London InterCommunity also has on staff specialty services of -Youth Outreach, Hepatitis C Care, Anonymous HIV Testing and an Identification Documentation Clinic – These are not directly attached to the Trans and Non-Binary Health Care Team, but are accessible to individuals engaged with us.

FUNDING CATCHMENT

We are limited by funding agreement to only servicing clients in our defined CHC catchment of London Ontario. Outside the city, you are welcome to contact our office, and we will advise if we are aware of alternate providers.
