We work within a health promotion framework and value inclusiveness, relationship building and harm reduction.

The health outreach team supports individuals with:

  • Addiction assessments and referrals to treatment
  • Harm reduction education and support
  • Housing referrals and loss prevention (available for our clients who are registered to our primary care team)
  • ID clinic
  • Ontario Works and ODSP applications (available for our clients who are registered to our primary care team)
  • Mail services

ID services at London InterCommunity Health Centre

We offer ID for individuals who are experiencing homelessness. Clients may be expected to bring in OW/ODSP statements to qualify for our program.
Our ID clinic is offered as a drop-in basis Mon-Thur 9-3:30 and Fridays 9-12, but booked appointments can be made as well.

Health cards– clients will be given a form to take to 100 Dundas to be processed, they will receive a temporary health card when handed in to 100 Dundas Service Ontario the same day, then mail their card to 659 Dundas st in 2-4 weeks time

Birth Certificates– our Same Day Community Workers at 659 Dundas St are trained to do all types of provincial and territorial birth certificates and usually take 8-10 weeks for Ontario and out of province can take 12-16 weeks to be mailed back to 659 Dundas St.

Replacement Permanent Resident cardsbooked appointments only.

Replacement Citizenship cardsbooked appointments only.

Affidavits– need to be booked appointments. Clients will need to know their SIN number to complete the form.

The Health Centre does not assist with the cost of Ontario Photo ID cards (visit Service Ontario, 100 Dundas St.), Social Insurance Numbers (visit Service Canada, 457 Richmond Street), Driver’s licence’s (Service Ontario) and/or Passports (Cherryhill Mall 301 Oxford St).
