Diabetes Education Program
We believe everyone living with diabetes can learn to live well with it. Our diabetes care team includes certified diabetes educators (Registered Dietitians and a Registered Nurse) who work together to support people with type 1, type 2, and prediabetes.
All are welcome. We specialize in serving individuals facing barriers such as language, culture, transportation, mental health, housing instability, or low income. Interpretation services available.
Services include:
- One-on-one education and support
- Diabetes care tailored for individuals with mental illness
- Self-management group programs (translation available)
- Supportive family involvement
Foot Care Program
Taking care of your feet is critical—especially for people living with diabetes. Our experienced foot care nurses provide high-quality care and education to help prevent serious complications.
Our foot care services include:
- Treatment for corns, calluses, fungal nails, ingrown toenails, cracked heels, and more
- Risk assessments for diabetes-related foot complications
- Routine foot screenings and self-care education
Who is eligible:
- People with diabetes who are clients of LIHC or a Team-Based Care program
- Individuals with mobility or vision challenges who cannot manage their own foot care
To access the service please complete the self-referral form or provider referral form and fax to 1-855-DIABETS (342-2387) or submit in person at 659 Dundas Street.
How to Access Foot Care Services:
Available by appointment only at our Dundas and Huron sites. Ask your care team for an internal referral.
