Our Primary Care team works together to keep you as healthy as possible. Care is provided by registered nurses, registered practical nurses, nurse practitioners, social workers, physicians, a community dietitian, and physiotherapists. Together, they provide treatment, health teaching, and support to help you reach your health goals.
We support people with both illnesses and chronic health conditions, and we have experience working with individuals who may face barriers to care. Our clients may be:
- Experiencing homelessness
- Beginning their journey to discover their gender identity
- Living with mental health or addictions challenges
- Starting a new life in Canada
No matter your circumstances, we strive to offer a warm, safe, and welcoming care experience.
How Our Primary Care Works
- We are not a walk-in clinic. Only rostered clients can book appointments with our doctors or nurse practitioners.
- We do our best to offer same-day appointments for urgent concerns.
- Each client is assigned a Primary Care team, and may see any member of that team depending on their needs.
- Spaces for Primary Care are limited. If you are not eligible, please contact Health Care Connect to find Primary Care providers in your area.
