Our registered dietitian supports individuals with complex health needs in reaching their nutritional goals. Diet impacts every part of our health, and proper nutrition can improve quality of life – especially for those living with chronic conditions.
We provide support for:
- Heart disease and digestive issues
- Osteoporosis and anemia
- Special diets (gluten-free, vegetarian)
- Emotional eating, meal planning, weight management
We also offer group classes on topics like label reading, heart health, and eating well on a budget.
To access this service clients must be part of LIHC primary care or referred through Team-Based Care. Ask your provider or care team to refer you.
