Aide pour l’obtention de pièces d’identité

Nous aidons les personnes en situation d’itinérance à obtenir des pièces d’identité. Pour bénéficier de ce service, les clients peuvent être tenus de présenter leurs relevés du programme Ontario au travail ou du Programme ontarien de soutien aux personnes handicapées.

The program helps bridge gaps in care for individuals with limited financial resources and complex health or social needs.

La clinique de santé pour les réfugiés offre des services sociaux et de santé aux réfugié·e·s pris·e·s en charge par le gouvernement, aux demandeur·euse·s d’asile, aux personnes protégées ainsi qu’aux réfugié·e·s parrainé·e·s par le secteur privé.

Programme de services complets pour les aînés

Le programme de services complets pour les aînés s’adresse aux personnes âgées francophones, à celles issues de l’immigration et à d’autres personnes vulnérables âgées de plus de 55 ans qui se sentent seules dans la communauté et qui pourraient avoir des problèmes de santé.

Seniors’ WrapAround

The objective of WrapAround program is to support senior individuals age 55 plus to live independently and integrated in the community. The program targets not only health concerns but also all aspects that could affect the life of a senior person (emotional support, financial, social, family support, spirituality, housing, educational, among others).

Systems Navigation & Intake Coordination 

Learning about all the programs and services at the Health Centre can feel overwhelming. Our Systems Navigators are here to help. They listen to each person’s story, assess their unique situation, and connect them with the supports that best meet their needs.

Refugee Health Clinic (Formerly Known as the Newcomer Clinic)

The Refugee Health Clinic Clinic provides health and social service supports to: government-sssisted refugees (GARs), refugee claimants, asylum seekers, protected persons , and private sponsored refugees

Health Outreach – Social Support for People Experiencing Homelessness 

Our staff meet people where they’re at in life – no matter their current circumstances. We work within a health promotion framework grounded in inclusiveness, relationship-building, and harm reduction.

Health Outreach Mobile Engagement

The H.O.M.E. Program brings health and social services directly to individuals who are unhoused, insecurely housed, or newly at risk of homelessness. Services are provided in shelters, encampments, and other locations across the city.
