The Gender Care Program provides gender-affirming medical and social supports through a wraparound team that includes nurse practitioner, a registered program nurse, social worker, a community health worker, and links to affirming specialists.

Services are provided at no cost for individuals living in London, Ontario

Our approach is grounded in the Informed Consent Model (ICM), which promotes patient-centred, self-determined care and aligns with the World Professional Association for Transgender Health (WPATH) Standards of Care.

Services Offered

  • Hormone Replacement Therapy (HRT) initiation and monitoring
  • Bloodwork for hormone levels and effects
  • Referrals for transition-related surgeries through the Ministry of Health
  • Post-surgical assessment and support
  • Mental health support related to transition
  • Puberty suppression (per Rainbow Health Ontario guidelines)
  • Supports with identification documents
  • Referrals to specialists (e.g., endocrinology, gynecology)
  • Clinical mentoring and regional provider support

Primary Care Collaboration

The program works in partnership with each client’s family doctor or nurse practitioner. We support aspects of trans health while your provider remains your main point of care. If you don’t have a family doctor, you may self-refer when intake reopens.
