Our Systems Navigators are committed to helping you find the supports and services that you need.  They can help connect you to internal or external community resources. 

If you are hoping to access our internal programs, we can arrange an Intake appointment with you.  While we do offer Primary Care, please know that we have limited spaces available.  If you are not eligible for our programs, please call Health Care Connect or use this link: https://www.ontario.ca/page/find-family-doctor-or-nurse-practitioner to see what Primary Care services are available in your area.

For help accessing external community resources or for more information, please contact one of our Systems Navigators and we will arrange for an appointment either virtually or in person to speak with you.

Our Systems Navigator at our Argyle site works differently as they focus on providing access to our programs and services for our Team-Care Referral clients or for our Newcomer Clinic clients.  They do not provide systems navigation services for the rest of the Health Centre.
