What is Team Care?

Team Care (formerly known as PINOT) provides health supports to the clients/patients of physicians not affiliated with an interprofessional team.

Our systems navigators assess the circumstances of each client/patient and then connects them to London InterCommunity Health Centre’s internal programs and services, at no cost, and to external resources.

With Team Care, everyone works together to better the health and well-being of the each client/patient.

 

What is the client/patient eligibility?

Team Care is intended for clients/patients who belong to the lowest income quintile (20%), and may identify with some of the following considerations:

  • Lives in East London
  • Has no private health insurance or has exhausted all EAP/benefits services
  • Experiences barriers in accessing services due to the social determinants of health

For a list and descriptions of all our programs and services click here. Please note, the Newcomer Clinic and Family Doctor programs are not available to Team Care clients/patients.

 

How can I access Team Care for my clients/patients?

Team Care is funded under the Ministry of Health and Long-Term Care through the London InterCommunity Health Centre. This allows us to provide clients/patients services at no cost to the client. Q: Will my clients/patients remain on my roster?

A: Yes, clients/patients will remain on your roster.

Q: Can the client/patient live outside of East London?

A: No, at the moment, our focus is on serving clients/patients who live in East London because research shows that people living in East London have the least access to team-based care. However we hope to expand services across London in the future.

Q: Are there any fees associated with your services?

A: No, there are no fees associated with the services provided by the London InterCommunity Health Centre.

Q: Can a community agency refer into Team Care?

A: The referral can be facilitated through a community agency by providing the physician’s contact to our team.

Q: Is this a pilot?

A: No, these dollars were added to base funding for the London InterCommunity Health Centre to expand services to physicians not affiliated with a team. This funding is ongoing.

Q: As a client/patient, can I register for Team Care?

A: No, if you are interested in participating in Team Care you must ask your physician about the program. If they decide to participate in Team Care, they will refer you accordingly.
