What is Team Care?

Team Care (formally known as PINOT) provides health supports to the clients/patients of physicians not affiliated with an interprofessional team.

Our systems navigators assess the circumstances of each client/patient and then connects them to internal programs and services, at no cost, and external resources.

With Team Care, everyone works together to better the health and well-being of the each client/patient.


What is the client eligibility?

Team Care is intended for clients/patients who belong to the lowest income quintile (20%), and may identify with some of the following considerations:

  • Lives in East London
  • Has no private health insurance or has exhausted all EAP/benefits services
  • Experiences barriers in accessing services due to the social determinants of health

Team Care is funded under the Ministry of Health and Long-Term Care through the London InterCommunity Health Centre. This allows us to provide clients services at no cost to the client.
